Here are the requirements set forth by LTO in requesting a duplicate copy of the vehicles’s Registration Certificate:

Request For Duplicate Certificate of Registration (CR)

Documents Required For Private Vehicles

  • Original Official Receipt (OR)
  • Affidavit of Loss
  • Certification from the Financing Institution that the Certificate of Registration (CR) is not in their possession (in case of Mortgaged/Encumbered Motor Vehicle)
  • Actual inspection of MV with duly accomplished MVIR
  • Taxpayer’s Identification Number (TIN)

Documents Required For For-Hire Vehicles

  • Original Official Receipt (OR)
  • Affidavit of Loss
  • Actual inspection of MV with duly accomplished MVIR
  • Certification from the Financing Institution that the Certificate of Registration (CR) is not in their possession (in case of Mortgaged/Encumbered Motor Vehicle)
  • Clearance from LETAS
  • Taxpayer’s Identification Number (TIN)
