If you ever lost your car’s plate for whatever reason or it has been detoriated and unreadable, you can request a duplicate plate from The Land and Transportation Office. The requirements below also applies for a request for the replacement of stickers.
Documents Required For Private Motor Vehicles
- Original Certificate of Registration (CR) and latest original Official Receipt (OR) of payment
- Actual inspection of MV with duly accomplished MVIR
- Clearance from LETAS
- Taxpayer’s Identification Number (TIN)

- Original Certificate of Registration (CR) and latest original Official Receipt (OR) of payment
- Affidavit of Loss
- Actual inspection of MV with duly accomplished MVIR
