After the topic of requesting duplicate plate number, this post is about how to request a duplicate registration certificate (CR) as well as the requirements for a duplicate copy or the Official receipt (OR).
Here are the requirements set forth by LTO in requesting a duplicate copy of the vehicles’s Registration Certificate:
Request For Duplicate Certificate of Registration (CR)
Documents Required For Private Vehicles
- Original Official Receipt (OR)
- Affidavit of Loss
- Certification from the Financing Institution that the Certificate of Registration (CR) is not in their possession (in case of Mortgaged/Encumbered Motor Vehicle)
- Actual inspection of MV with duly accomplished MVIR
- Taxpayer’s Identification Number (TIN)
Documents Required For For-Hire Vehicles
- Original Official Receipt (OR)
- Affidavit of Loss
- Actual inspection of MV with duly accomplished MVIR
- Certification from the Financing Institution that the Certificate of Registration (CR) is not in their possession (in case of Mortgaged/Encumbered Motor Vehicle)
- Clearance from LETAS
- Taxpayer’s Identification Number (TIN)
